Support Request Criteria

PLEASE READ THOROUGHLY BEFORE COMPLETING YOUR SUPPORT REQUEST!

  • Support can be requested for children in Montana birth and 18 years old.

  • Financial support is provided only for medical travel related expenses (lodging, gas, meals, prescriptions not covered by insurance, etc.).

  • Support requests include three steps:

    • Step 1 - Application

      • The application must be completed by a parent, guardian, or family member. Applications completed by social workers will not be accepted.

      • Applications must be accurate and complete.

    • Step 2 - Consent for Verification

    • Step 3 - Written & Signed Referral

      • A referral letter from your doctor or social worker, on professional letterhead, must also be emailed to lindsay@jadynfred.org and include:

        • Child’s need for travel support

        • Upcoming travel dates or ongoing medical plan

        • Diagnosis

        • Doctor’s notes or H&P

  • Support requests will be denied if required information is missing or inconsistent (e.g., mismatched dates, incomplete provider verification, or missing consent forms).

  • The Jadyn Fred Foundation may support any one child a total of four (4) times.

    • Families may reapply on the anniversary date of their last application.

Step 3. Referral from Physician or Social Worker

Email referral to: lindsay@jadynfred.org. Referral must be on the referring physician or social worker’s professional letterhead and include:

  • Child’s need for travel support

  • Upcoming travel dates or ongoing medical plan

  • Child’s Diagnosis

  • Doctor’s notes or H&P
