Applying is as easy as:

1. Application

   Fill out the online application and sign the Release of Medical and Insurance Records

2. Documentation

   Provide adequate documentation from the attending physician or Social worker confirming the need for specialized attention.

3. Communication

   Wait for us to reach out to you to let you know the approval status of your application* We will reach out to you within 5 business days.

* Applicants are eligible to receive assistance once per year, up to four years. After an applicant has been supported four times they are no longer eligible to receive assistance from the Jadyn Fred Foundation.
